Forms and How-To Guides

Assisted, Wheelchair, and Stretcher Transportation Requests

Transportation requests for Modes 4, 5, and 6 can be submitted via the Atrezzo Provider Portal or phone. Please note that a medical necessity questionnaire must be completed.

Screening for the medical necessity questionnaire can be completed by submitting the request via the Atrezzo Provider portal (24hrs/day) or by calling the customer service team at 844-681-8144 during business hours.

Protected Transportation

Protected Transportation is a state administered nonemergency medical transportation (NEMT) transport level of service. It is intended for use by a member who received a transportation level-of-service (LOS) assessment, and whose assessment determined that other forms of transportation are not appropriate. The member must require transport by a provider who meets both of the following criteria:

Mental health crisis teams certify the member for protected transportation services using the Protected Transport – Level-of-Service (LOS) Assessment (DHS-6715) (PDF). Submit the form to Acentra via the Atrezzo Provider Portal (preferred) or via fax 844-673-8033.

Beginning January 1, 2026, Acentra will reduce turnaround times to 10 days for pended cases and 72 hours for expedited cases, with incomplete expedited requests now resulting in rejection. Expedited reviews only apply when delays could seriously impact a member’s life, health, function, or create risk of severe pain or emergency. A new Member Portal will launch, allowing Medicaid recipients to view authorizations and submit appeal documents. Providers are encouraged to use the Atrezzo Portal for faster, easier documentation submission and case communication. For more details see the attached newsletter below.
