DME providers who need wheelchair parts can now get an authorization within 24 hours for approved cases by submitting via the portal and filling out a questionnaire! Applies to codes:

HCPCS Codes: K0108, E0953, E0955 – E0957, E0960, E0966, E1028[RM1] [RM2], E2231, E2601, E2604 – E2609, E2610 – E2617, E2620 – E2624, E2291 – E2294

The provider will be required to complete a 4-question questionnaire based off of InterQual Criteria which will contain the questions regarding the following information:

1. Request needs to identify reason for replacement
2. Only covers the cushion or seating system for manual wheelchairs and power wheelchairs with sling or solid seats and backs.
